HollywoodPrize: User Account Creation

As you enter this Contest or RSVP to this Event, you will simultaneously create a HollywoodPrize.com user account. This user account allows you to enter future promotions powered by HollywoodPrize without having to fill-out this form ever again. After completing this form, simply login to HollywoodPrize and click on MyContests and Add A Contest to enter future Contests or Events. Then simply choose the appropriate Contest or Event you wish to enter. You will not need to do so for this Contest or Event however as your entry will already be recorded upon clicking the submit button at the bottom of this form.

You will also receive 1 rewards point for entering this Contest or Event. You can earn 1 additional point for each Contest or Event you enter. Earn enough Points and HollywoodPrize (not the Sponsor) will send you an additional free gift. You can also earn 1 additional point for each Friend (with a valid email and US Postal address) you refer to this promotion or to HollywoodPrize.com. Any entrant caught creating bogus accounts with false email / Postal addresses will not be rewarded. Prize winners must provide prove proof of residency. Duplicate Postal addresses in our system will not be counted.

When you are creating your user name, please be very careful! The user name cannot be changed once created. The user name also creates your own personal page on HollywoodPrize called a MySpotlight page (for Entertainment Consumers) or a MyPortfolio Page (for Artists) and provide you with the following URL: http://www.hollywoodprize.com/myusername

You can choose Consumer / Fan Account or Artist Account under Contestant Account Type on this form.

You can then add Friends (called Key Holders) and comment on other personal pages or send emails (called Coins) through the HollywoodPrize system to those you have connected with.
